Case Study #1

This case involves replacement of a congenitally missing tooth and cosmetic improvement of 6 upper front teeth in an 18 year old woman. Prior to this, this patient had orthodontic treatment, but the result was less than ideal.

This case demonstrates the replacement of a severely decayed and broken upper right canine tooth with an immediate loading implant. The implant surgery was done without opening the gum tissue (flapless approach), to prevent damage to or possible shrinkage of the tissue.

This x-ray shows the extent of decay on the tooth which has penetrated to the root portion. Patient’s dentist had decided the tooth was hopeless and referred her for extraction and implant placement.
Tooth was extracted without any incision and with minimal trauma to gum tissue. This type of extraction require advanced knowledge and experience of the surgeon. The implant was placed immediately following extraction.

A temporary crown was fabricated and placed. This temporary crown will be replaced with a permanent one 3 months later. By applying the latest in minimally-invasive surgical principles and as a result of his extensive experience and knowledge, Dr. Khazian performed this procedure in 65 minutes.

Note the clean and esthetic result shown in this post-operative picture. Any deviation from such principles will result in permanent damage to gum tissue and produce an unpleasant appearance when smiling. These types of damages are extremely difficult to correct and often times will never restore an acceptable result.

This case demonstrates a simultaneous extraction of a tooth and placement of an implant followed by immediate restoration by a temporary tooth. Root fracture has caused infection in the gum which is clearly visible in this picture.

This close up picture shows the tooth and the infected gum. Root fracture is a condition requiring immediate extraction of the tooth. Following extraction, the tooth had to be replaced immediately. This is an ideal indication for an immediate loading dental implant.

The tooth was in the upper front of the mouth which is considered an esthetic zone. Therefore, extreme care was taken to prevent damage to the gum tissue. The tooth was extracted without opening the gum (flapless approach)

Implant was placed. This surgery was done with absolute minimum trauma to the gum tissue. Preservation of the gum architecture in this esthetic area is of utmost importance. Any damage done to the gum tissue during surgery will cause permanent esthetic malformation which would be extremely difficult or impossible to correct.

A temporary crown is placed on implant immediately following it’s placement. This temporary crown will be replaced with a permanent porcelain one in 3 months. This procedure was done in less than an hour and with minimal trauma. Patient is able to return to work or other daily activities in the same day.

This case demonstrates replacement of a missing upper front tooth in a young male (23 years old). Obviously esthetics is the main concern in this case. As shown in this picture, the remaining upper front teeth have 2 tone colors, with the lower half being slightly grayish. This creates an esthetic challenge in color matching. Care should be taken to blend the final tooth so that it will be indistinguishable and natural looking.

The ultimate choice for replacement of such missing tooth is a dental implant. The implant has already been placed by Dr. Khazian.

This case demonstrates replacement of a missing tooth on upper right molar area. An implant have already been placed by Dr. Khazian. This implant was placed at the time of tooth extraction. The healing period was 4 months and is now ready for restoration.

The implant is already restored with a porcelain crown. Note the excellent adaptation of porcelain to the gum tissue which makes it indistinguishable from natural teeth.

The crown was made slightly whiter than other teeth to provide a perfect color match after tooth whitening is done.

Another photo shows a perfect integration of implant tooth into the upper arch. This patient is able to brush and floss this implant tooth exactly the same way as in natural teeth. An implant placed in this manner and properly maintained will usually last a life time.
